I apply to join Usdaw. As a member of Usdaw I undertake to abide by the rules and regulations of the Union and to pay contributions regularly.
The home address you give is the address that Usdaw will use for balloting purposes. I give my explicit consent to the processing of data under the Data Protection Act 1998 as specified below.
The responsibility for keeping payments up to date rests with the member.
If paying contributions via Payroll
I hereby authorise my employers for the time being, or their representatives, to deduct from my salary or wages the amount of contributions payable by me under the Union’s Rules, as amended from time to time. I also authorise the deduction of any arrears which may accrue during my employment. I consent to the Union sharing my personal data with my employers, or their representatives in order to process my deduction contributions. I also authorise my employers, or their representatives, to share my information with Usdaw in order to process and maintain my Union membership and where appropriate that my employers, or their representatives, shall notify the Union of any future changes in my home address and email to enable the Union to maintain a register of the names and proper addresses of its members.
If paying contributions by Direct Debit
By applying to join Usdaw online I (the applicant) am confirming that I am the owner of the specified bank account. Usdaw’s online join facility will undertake a search with Experian for the purposes of verifying your identity in order to comply with the Union’s legal obligation under BACs processing. Experian may check the details you supply against any particulars on any database (public or otherwise) to which they have access. - This Guarantee is offered by all banks and building societies that accept instructions to pay Direct Debits.
- If there are any changes to the amount, date or frequency of your Direct Debit Usdaw will notify you 5 working days in advance of your account being debited or as otherwise agreed. If you request Usdaw to collect a payment, confirmation of the amount and date will be given to you at the time of the request.
- If an error is made in the payment of your Direct Debit by Usdaw or your bank or building society you are entitled to a full and immediate refund of the amount paid from your bank or building society.
– If you receive a refund you are not entitled to, you must pay it back when Usdaw asks you to.
- You can cancel a Direct Debit at any time by simply contacting your bank or building society. Written confirmation may be required. Please also notify us.

Online Courses
Usdaw will process your personal data as part of its legitimate activities in providing trade union services and benefits in accordance with the Union’s objectives stated in the rule book and to perform obligations under statute or contract. Privacy Notice for Surveys, Petitions & Pledges
The data you are providing us will be used for statistical purposes in supporting our Campaign as well as updating any contact details held on your membership record (if you are an Usdaw member). This is in pursuit of the Union's legitimate activities stated in the Objects in the Rule Book and in order to fulfil our contractual obligations to you as a member. This processing involves your personal data and special personal data. The data that we collect about you here will be stored securely. The survey responses will be retained for the duration of the campaign and the membership record will be retained for the duration of your membership plus 20 years in line with our membership record retention schedule.

Cancellation of your Usdaw Membership
You will no longer be entitled to our workplace support or member benefits and services. To process your cancellation, form CAN1 will need to be completed and returned to us. Any cancellation will become effective from the next contribution period with no pro rata refunds being made for prior periods.
